Here is a modern version of the Apostles' Creed, also known as the Nicene Creed, with a prayer:

The Apostles' Creed (Modern Version)

I believe in God, the Father Almighty, Creator of heaven and earth. I believe in Jesus Christ, his only Son, our Lord, who was conceived by the Holy Spirit, born of the virgin Mary, suffered under Pontius Pilate, was crucified, died, and was buried; he descended to the dead. On the third day he rose again; he ascended into heaven, and is seated at the right hand of God the Father; he will come again to judge the living and the dead.

I believe in the Holy Spirit, the holy catholic Church, the communion of saints, the forgiveness of sins, the resurrection of the body, and the life everlasting.

Prayer

Dear God, we come before you today with humble hearts, acknowledging your sovereignty and majesty. We confess that you are the Creator of all things, and that you have given us life and breath.

We believe in Jesus Christ, your only Son, who came to save us from our sins. We believe that he was conceived by the Holy Spirit, born of the virgin Mary, and that he lived a sinless life. We believe that he suffered and died on the cross, and that he was buried and rose again on the third day.

We believe in the Holy Spirit, who guides and empowers us to live according to your will. We believe in the holy catholic Church, which is the body of Christ, and in the communion of saints, who are united in their love for you.

We ask for your forgiveness for our sins, and we pray for the resurrection of our bodies and the life everlasting. May your will be done on earth as it is in heaven. Amen.
